背景：兒童急診是醫療照護關注的重點。兒童相對於成人是醫療照護的弱勢族群。不論疾病種類或急救流程的醫療照護，兒童病患都不同於成人，美國國家醫學研究院（Institute of Medicine；簡稱IOM）自1993年開始完整檢視兒童急診照護，2006年強調兒童急診依然是”Growing Pain”，許多兒童急診醫療的不足及不均是政策關心的重點，且與整個健康照護體系息息相關。過去關於兒童急診醫療利用和品質的趨勢變化研究了解有限，特別是在台灣等新興國家。台灣健保醫療體系為單一保險人的社會醫療保險體系，費用低可近性高。當許多國家正在計劃或正在進行醫療保健改革，並朝著普及全民醫療保險的方向發展，所以

探討全民保險體系下兒童急診醫療利用和品質的現況及變化，對兒科急診照護的影響是很重要的。目的：第一部分的研究探討醫療體系對兒童急診醫療行為的影響，分析在相同醫療提供者的醫療服務下，比較中國與台灣不同醫療體系中兩個兒童急診環境下治療常見呼吸系統疾病的差異，觀察醫療照護體系如何影響兒童急診醫療品質。第二部份評估2000到2015年台灣兒童急診醫療服務的醫療利用和品質趨勢變化。方法：第一部分的研究資料來源採用多中心的醫院資料庫，取樣十八歲以下的兒童急診就診，針對同一群兒童急診醫師在中國廈門與台灣基隆兩家教學醫院，進行匯集橫斷面研究，分析兒童急診常見呼吸系統疾病的品質指標，比較不同的醫療體系對同一組兒童

急診醫生執業模式的影響。第二部分的研究我們使用台灣全民健康保險資料庫，將從全人口資料庫中2000年、2005年、2010年、和2015年全年十八歲以下兒童的所有急診就診趨勢，評估不必要的輕症就診和急診返診相關的品質指標。結果：台灣的醫療體系與中國比較方面，在控制相同的醫療提供者，並調整重要的病患特徵之後，台灣的兒童急診常見呼吸道疾病的照護有較佳的品質。兒童急診氣喘的品質指標24到72小時返診風險在中國顯著高於台灣7.76倍；兒童急診細支氣管炎的品質指標抗生素的使用在中國顯著較高(勝算比 2.19；95% 信賴區間 1.46-3.28)，同時看到胸部X光片的使用在中國顯著較低 (勝算比 0.21

; 95% 信賴區間 0.17-0.28)。至於台灣兒童輕症急診的醫療利用在族群就診趨勢上並沒有上升；在標準化人口結構後每百人族群的兒童輕症急診就診從2000年10.32 降到2015年 9.04 (12.40%)。雖然總兒童急診就診從每百人族群2000年23.80 升到2015年29.07 (22.14%)，我們的研究中同時分析到在兒童急診持續上升的檢驗、檢查和處置的比率趨勢，意味著家屬和病患有其他的需求，兒童急診醫療利用趨勢的上升不是輕症急診的醫療利用。兒童急診的醫療品質趨勢，返診率和急性腹症的返診率都持續下降；標準化人口結構後三日返診率從2000年6.01% 降到2015年 5.67%

(降幅5.65%); 但三日返診在同一天的返診率呈現上升趨勢(2000年1.46%; 2015年1.83%; 升幅25.34%)。兒童急診急性腹症的返診率在返診後診斷急性盲腸炎的比率從2000年0.026% 降到2015年 0.014% (降幅46.15%)，返診後診斷腸套疊的比率從2000年0.013% 降到2015年0.008%，(降幅38.46%)。結論：即使是相同的醫療提供者，在不同的醫療體系下醫療行為也會不同，進而影響兒童急診醫療品質。在台灣的健保制度下全面涵蓋了急診服務，並且分攤費用低，但兒童不必要的輕症急診醫療利用並未顯著增加，更重要的是，在2000年至2015年間，沒有觀察到兒

童急診照護品質惡化的跡象。政策意涵：國內外急診就診趨勢持續上升，兒童急診就診量佔將近四分之一急診就診，超過一半的就診發生在小於五歲的小孩，我們的研究顯示在中國和台灣，相同的醫療提供者其醫療行為會受到不同醫療照護體系的影響，因素包括有保險涵蓋、家屬認知、和基層醫療等，進一步影響兒童急診的醫療品質。我們的研究結果顯示，台灣兒童醫療照護的經驗呈現完整的涵蓋，在兒童急診的醫療服務上提供有需要且持續改善品質的兒童急症照護，對兒童急診照護有幫助的完整醫療系統，涵蓋足夠的給付、完整的基層醫療和有效醫療的衛教。對於兒童急診醫療不足的地區和醫院層級，應關注其醫療品質並建議加強與醫學中心間的院際交流以強化區域和地

區院所的照護品質，或可考慮地區內新的醫療整合模式，降低同日就診的醫療不便。
